Can depression cause erectile dysfunction?
NIDDK says “some mental health or emotional issues can either cause ED or make it worse,” including “anxiety, depression, or stress.” That answers both “does depression cause ED?” and “can depression cause impotence?”—MedlinePlus lists impotence as another name for erection problems.
Depression is a possible explanation, not a diagnosis of your ED. NIDDK also lists conditions affecting blood vessels, nerves or hormones. Use our causes of ED guide to prepare questions rather than assume every erection problem is from low mood.
The two-way link: what the research shows
Depression and ED are associated in both directions. An odds ratio above 1 means higher odds in the group studied; it is not the percentage of men affected or your personal risk. The Liu meta-analysis could not establish which condition caused the other.
| Evidence and year | Finding | What it cannot tell you |
|---|---|---|
| Depression → ED · meta-analysis · 2018 | Pooled odds ratio 1.39 (95% CI 1.35–1.42) across 48 studies in 46 publications. Liu et al. | Large heterogeneity (I² 93.6%); the authors state the analysis could not determine the direction of causality. |
| ED → depression · same meta-analysis · 2018 | Pooled odds ratio 2.92 (95% CI 2.37–3.60) across 6 studies in 5 publications. Liu et al. | Far fewer studies than the other direction; an association, not proof that ED caused anyone’s depression. |
| Sildenafil in men with depressive symptoms · randomized trial · 2001 | 152 men, 12 weeks: 48 given sildenafil vs 10 given placebo met conservative ED-response criteria. Hamilton depression scores fell by a mean 10.6 in ED responders vs 2.3 in nonresponders. Seidman et al.; Viagra (sildenafil) label | Mild-to-moderate depressive illness only; mood change was compared by ED response, not tested as a depression treatment. |
| Penile implant · health-record study · 2026 | Among men with at least mild symptoms, PHQ-9 depression scores fell 23.5% after implant surgery; anxiety (GAD) scores did not improve. SMSNA summary of Khera et al. | One observational study; 255 men had usable questionnaires; most co-authors list Boston Scientific affiliations. Association, not proof. |
Sources: Liu meta-analysis; Seidman trial; Khera study record; SMSNA patient news, Oct 27, 2026; Viagra (sildenafil) label. Accessed Oct 28, 2026.
The meta-analysis authors drew a practical conclusion: men reporting ED “should be routinely screened for depression,” and men with depression symptoms “should be routinely assessed for ED” (Liu et al.).
Mood-related or physical ED: clues to bring to your visit
Morning erections are a clue, not a test for depression. MedlinePlus says erections during sleep or on waking suggest a cause is likely not physical. A clinician still needs your health and sexual history.
| What you notice | What the source says | What to do with it |
|---|---|---|
| Erections still happen in the morning or during sleep | “It is likely not a physical cause” (MedlinePlus) | Mention it; it points away from, but does not rule out, a physical cause. See no morning wood. |
| Doubt, fear of failure, stress or anxiety around sex | Listed as emotional causes of ED (MedlinePlus) | Read performance anxiety and ED; counseling can help (NIDDK). |
| Diabetes, heart or blood vessel disease | Listed among physical causes (NIDDK) | Physical and mood causes can coexist; report both. |
| You take antidepressants or other medicines | Some antidepressants can affect sexual function; the Zoloft (sertraline) label lists ED. | Bring the medicine name and when erection problems began; see the next section. |
Sources: MedlinePlus erection problems; NIDDK causes; NIDDK treatment. Accessed Oct 28, 2026.
A clinical assessment considers your medical, sexual and mental health history; a nighttime erection test may help investigate a physical cause. Read ED symptoms for what to report.
Could your antidepressant be the cause?
Depression and its treatment can both enter the picture. For example, the Zoloft (sertraline) label lists erectile dysfunction and says not to stop it without talking to your provider. Tell the prescriber whether erection problems started before or after treatment; our antidepressants and ED guide covers individual medicines.
Does treating ED help depression?
Better erections and better mood can occur together; these studies do not establish ED treatment as depression treatment. Keep depression care part of the conversation.
The Oct 27, 2026 news: SMSNA’s patient summary describes one observational health-record study by Khera et al., J Sex Med 2026;23(9). Depression questionnaire scores improved after penile implantation; anxiety scores did not. The figures and limited questionnaire sample appear in the evidence table above. Most co-authors listed a Boston Scientific affiliation; the results show an association, not proof of benefit for depression.
Implants are a later option: NIDDK says most people with ED do not need surgery and describes it after other options have failed. This study is not a reason to jump from depression symptoms to implant surgery.
What about pills? The Seidman randomized trial found sildenafil effective for ED in men with mild-to-moderate depressive illness, with mood improvement associated with ED response. The Viagra (sildenafil) label indicates treatment of ED. The trial does not establish a depression indication or evidence for a compounded combination.
What each path includes
Choose the next step by what you need help with now. Bring both mood and erection concerns to the appointment; an ED visit does not replace mental health care.
| Your priority | Starting point | What it includes | Boundary |
|---|---|---|---|
| I am in crisis or thinking about suicide | 988 Suicide & Crisis Lifeline | Call or text 988, or chat, 24/7; call 911 if life is in danger (NIMH) | Comes before any ED question |
| I want help with low mood | Primary care or a mental health professional | Depression assessment and treatment; counseling can also address stress about sex (NIDDK) | ED may need separate attention |
| I want to understand recurring ED | Clinical ED assessment | Medical, sexual and mental health history; tests as indicated (NIDDK) | Mood and physical causes can coexist |
| I want to discuss an ED prescription | Online visit or in-person clinician | Sildenafil or tadalafil if appropriate (Viagra (sildenafil) label; Cialis (tadalafil) label) | Both require sexual stimulation; Prescription required |
| I want to know whether Mendoxa fits | Mendoxa online ED visit | Online review by Independent licensed providers, including your antidepressants and other medicines | Not depression care; compounded drugs are not FDA-approved |

Bring this to your online ED visit
- When erection problems began and whether morning erections still occur.
- How your mood has changed and any current depression care.
- Your prescription medicines, over-the-counter medicines and supplements.
- Your main question: “Could mood, medication or a physical condition be contributing?”
This checklist follows the history described by NIDDK and MedlinePlus. For background, read our sildenafil guide, tadalafil guide and latest ED treatments; the Viagra (sildenafil) label and Cialis (tadalafil) label describe the separate approved products.

When to get help right away
If you are thinking about suicide or feel unsafe, call or text 988 now, or chat at 988lifeline.org. The 988 Suicide & Crisis Lifeline is “available 24/7/365” and “is for everyone.” In a life-threatening emergency, call 911 (NIMH).
Outside a crisis, arrange care when low mood affects daily life or erection problems concern you. NIMH explains that depression can affect thinking, feeling and daily activities; MedlinePlus describes assessment for both erection problems and emotional concerns.
Safety checks before ED medication
- Sildenafil and tadalafil are contraindicated with organic nitrates or riociguat (Viagra (sildenafil) label; Cialis (tadalafil) label).
- An erection lasting more than 4 hours needs immediate medical care (Viagra (sildenafil) label; Cialis (tadalafil) label).
- Stop and seek care for sudden vision loss or sudden decrease or loss of hearing (Viagra (sildenafil) label; Cialis (tadalafil) label).
- Common adverse effects include headache and flushing with sildenafil, and headache, indigestion and back pain with tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label).
- For its injectable Parkinson’s product, the APOKYN (apomorphine) label contraindicates use with 5HT3 antagonists such as ondansetron and warns about nausea and fainting.
These labels describe separate approved products, including an apomorphine injection for Parkinson’s disease; they do not establish the safety of a compounded ED tablet. Review the full medicine list with the prescriber. See ED medication and nitrates; the Viagra (sildenafil) label and Cialis (tadalafil) label explain those contraindications.
If ED is the problem you want treated now
Start with an online ED visit where a clinician goes over your medicines, including antidepressants. Independent licensed providers assess whether treatment is appropriate. Prescription required. Mendoxa does not provide depression care; keep your mental health clinician involved. Mendoxa ED care explains the process.
Use the visit to ask what is appropriate for your health history and medicines, and what needs an in-person assessment. The compounded product and costs are explained below.

FAQ
Does depression cause ED in men?
Yes. NIDDK lists depression among issues that can cause ED or make it worse. That does not establish the cause of your erection problems; physical conditions and medicines also deserve assessment.
Can erectile dysfunction cause depression?
The two are linked in that direction too: a 2018 meta-analysis reported a pooled odds ratio of 2.92 for ED exposure and depression risk, based on 6 studies. The authors note the data cannot prove which condition causes the other.
How do I know if my ED is from depression or something physical?
You cannot be sure on your own. MedlinePlus says morning or sleep erections suggest the cause is likely not physical, while NIDDK describes a nighttime erection test clinicians can use to check for a physical problem.
Does Viagra work if you are depressed?
Yes, it can help some men with ED and depression: the Viagra (sildenafil) label reports effectiveness in patients with a history of depression and in those taking antidepressants. This does not mean it is appropriate for everyone; the Viagra (sildenafil) label contraindicates use with nitrates. A prescriber needs to review your medicines and health history.
Will treating my ED improve my depression?
Some studies associate ED improvement with better mood, but they do not establish ED treatment as a treatment for depression. The Seidman sildenafil trial studied ED response in men with mild-to-moderate depressive illness; the Viagra (sildenafil) label indicates ED treatment. Keep depression care separate.
Can I get ED treatment online if I take antidepressants?
An online ED visit can be a starting point for a medication review; a clinician decides whether online care and treatment are appropriate. Tell them about all antidepressants and other medicines. Independent licensed providers assess Mendoxa visits. Prescription required. Mendoxa’s compounded tablet is not FDA-approved.

How we wrote this
Written by the Mendoxa editorial team · Sources: DailyMed/FDA. Medical facts on this page come from the FDA-approved labels on DailyMed and Drugs@FDA; each drug name links to its label. We checked the sources on Oct 28, 2026 using live NIH, PubMed and DailyMed/FDA pages and the supplied source copies. The evidence table separates associations from treatment results; study figures come from PubMed abstracts, except the implant findings, which come from the supplied SMSNA patient summary. The implant article has no PubMed abstract. These studies do not establish that ED medication treats depression. Separate approved-product labels do not establish the safety or effectiveness of Mendoxa’s compounded combination.
