SMS Terms
Last updated: October 6, 2026
These SMS Terms govern the Mendoxa text messaging program. They sit alongside our Terms of Use and Privacy Policy. Program operator: Hugo Bazan LLC, DBA Mendoxa, 651 N Broad St Suite 201, Middletown, DE 19709.
Program
Mendoxa sends transactional SMS to people who opt in at try.mendoxa.co (intake / patient portal). Messages cover visit status, shipping and delivery, account notices, identity-document retry, and care-coordination logistics. We do not send marketing or promotional SMS. We do not send protected health information, medication names, diagnoses, or lab results by text. Clinical details stay in the patient portal.
Consent is not required to buy
Consenting to receive SMS messages is not a condition of purchase or use of Mendoxa services. You can complete a visit and receive care without joining the SMS program.
How you opt in
You opt in by checking a box on the Mendoxa intake or account form, or by texting START to our sending number after we have that number. The checkbox language is:
I agree to receive transactional text messages from Mendoxa about my visit, shipping, and account. Message frequency varies. Msg and data rates may apply. Reply STOP to opt out, HELP for help. Consent is not a condition of purchase. Privacy · SMS Terms.
We keep records of the opt-in date, method, language shown, and program consented to.
Frequency and rates
Message frequency varies. You may receive up to 8 messages per month. Message and data rates may apply depending on your mobile carrier and plan.
Opt out
Reply STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT, or OPTOUT. We also honor opt-out requests made by any other reasonable method, including email to [email protected]. You will receive a single confirmation message. No further SMS will be sent unless you opt back in (reply START).
