ED symptoms: the checklist
Use this checklist to describe the problem at your visit. ED is sometimes called impotence; the same erection symptoms apply (NIDDK).
- Trouble getting an erection when you want to have sex (MedlinePlus).
- Trouble keeping it: the erection fades before or during sex (NIDDK). See can’t stay hard and losing an erection during sex.
- Reduced firmness: an erection that is not firm enough for intercourse (MedlinePlus).
- Sometimes but not others: erections sometimes, but not every time you want sex (NIDDK).
- Fewer morning or nighttime erections: a detail to report when describing your erections (MedlinePlus). Read no morning wood.
- Lower interest in sex: often listed alongside ED, but it is a different symptom; erection problems do not usually affect sex drive (MedlinePlus).
Early signs of erectile dysfunction vs normal occasional trouble
An occasional erection problem does not by itself establish ED. Look for a recurring change in getting hard, staying hard or firmness (MedlinePlus). Tell the provider whether it began suddenly or gradually and whether it happens with a partner, alone or in both settings.
Morning erections are a clue, not a home diagnosis. Their presence makes a physical cause less likely according to MedlinePlus; their absence alone does not identify a cause. Stress and anxiety can contribute (NIDDK). Read about no morning wood and performance anxiety and ED.
What each symptom may mean and who to see
The table links each pattern to a possible meaning and a sensible first contact. It is a guide for the conversation; only an assessment can identify your cause.
| Symptom | What it may mean | Who to see first |
|---|---|---|
| Erections sometimes, not every time | Stress, anxiety or depression can cause or worsen ED (NIDDK) | Primary care or an online licensed provider; counseling may be suggested if emotional issues play a part (NIDDK) |
| Erection fades before sex is over | A core ED symptom: an erection that does not last long enough for sex (NIDDK) | Primary care or an online licensed provider |
| Less firm, or no erections at all | Possible causes include blood vessel disease, high blood pressure, diabetes, nerve or hormone problems (NIDDK) | Primary care for a health check, including heart risk factors (Thompson et al.) |
| Morning or night erections have stopped | A clinician may ask about nighttime erections or consider testing (NIDDK diagnosis) | Primary care or urologist; a nighttime erection test is one option (NIDDK) |
| Low interest in sex | Not usually caused by ED itself; low testosterone can reduce sex drive (MedlinePlus) | Primary care; ask whether blood tests are needed |
| Curve, hard lump or pain with erections | Possible Peyronie’s disease, scar tissue under the skin of the penis (NIDDK) | Urologist (NIDDK) |
| Started after injury or prostate surgery, or with back or belly pain or urination changes | Reasons to contact a provider (MedlinePlus) | Your surgeon, urologist or primary care |
| Chest pain or pressure | A possible heart attack symptom (NHLBI) | Call 911 |
| Erection lasting more than 4 hours | Needs immediate medical assistance (Viagra (sildenafil) label) | Emergency care now |

Red flags: when ED symptoms need urgent care
- Sudden erection trouble with chest pain, heaviness or discomfort, during sex or at any time: call 911, even if you are not sure it is a heart attack (NHLBI). The Viagra (sildenafil) label notes a potential cardiac risk of sexual activity in people with existing cardiovascular disease.
- An erection lasting more than 4 hours, painful or not, needs immediate medical care (Viagra (sildenafil) label; Cialis (tadalafil) label).
- A new curve, hard lump or pain during erections: see a urologist about possible Peyronie’s disease (NIDDK).
- ED after an injury or prostate surgery, or with low back pain, abdominal pain or a change in urination: contact a provider (MedlinePlus).
- Sudden vision loss or sudden decrease or loss of hearing while taking an ED medicine: stop and seek care (Viagra (sildenafil) label; Cialis (tadalafil) label).
For recurring symptoms without these urgent signs: arrange an assessment. You do not have to wait until erections fail every time (MedlinePlus).
Is ED an early sign of heart disease?
Sometimes, which is why new ED deserves a health check. MedlinePlus says ED may be a sign of health problems such as diabetes or heart disease. The studies below describe groups of men, not your personal risk.
| Study and year | Finding | What it cannot tell you |
|---|---|---|
| Prostate Cancer Prevention Trial placebo group · 2005 | The placebo group enrolled 9,457 men aged 55+; in those initially free of ED and cardiovascular disease, new ED was linked to later cardiovascular events: hazard ratio 1.25 (95% CI 1.02–1.53). Thompson et al., JAMA | A modest group-level association in older men; it does not mean ED will lead to a heart event for you. |
| 300 patients with chest pain and confirmed coronary disease · 2003 | 49% had ED; in 67% of them, ED began before heart symptoms, by a mean of 38.8 months. Montorsi et al., Eur Urol | A selected group already had coronary disease; this cannot estimate heart disease risk for everyone with ED. The authors called for controlled studies. |

Ask whether you need a cardiovascular risk assessment, rather than assuming ED means heart disease (Thompson et al.). See ED and blood pressure and ED and diabetes.
The IIEF-5 (SHIM) questionnaire: how symptoms are scored
The IIEF-5 (SHIM) helps describe erection difficulties and their severity. This five-question tool covers erectile function and intercourse satisfaction (Rosen et al., 1999; SHIM terminology). It does not tell you what is causing the problem.
In the original validation study, scores ranged from 5 to 25; 22–25 indicated no ED and 5–7 severe ED. A cutoff of 21 best distinguished the study groups, with sensitivity 0.98 and specificity 0.88 (PubMed abstract). These are study results, not a substitute for an examination. Ask your provider to administer and interpret the questionnaire.
We do not reproduce the questionnaire items. Diagnosis also uses health and sexual history, examination and testing when appropriate (NIDDK).
What each path includes
Match the first step to your symptoms. These paths organize the conversation; they are not a self-treatment algorithm.
| Path | Fits when | What it includes | Boundary |
|---|---|---|---|
| Emergency care | Chest pain, or an erection lasting more than 4 hours | Call 911 for suspected heart attack; get emergency care for a prolonged erection (NHLBI; Viagra (sildenafil) label) | Not for routine ED questions |
| Primary care visit | New or worsening ED, or heart, blood pressure or diabetes risk factors | Medical, sexual and medicine history, exam and blood tests as needed (NIDDK) | May refer you on for curvature, injury or surgery-related ED |
| Urologist | Curve, lumps or pain; ED after pelvic surgery or injury | Specialist exam; urologists diagnose ED and Peyronie’s disease (NIDDK) | Can investigate specialist concerns |
| Counseling | Situational ED, anxiety or stress about sex | Help lowering anxiety and stress about sex, sometimes with a partner (NIDDK) | Physical causes still need a medical check |
| Online visit for an ED prescription | Ongoing ED without red flags | A licensed provider reviews history and medicines; options may include sildenafil or tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label) | Prescription required; red flags need in-person care |

What to bring to the visit
Write down when symptoms began, how often they occur, whether you lose firmness before or during sex, morning erections, and any change in desire. Bring a complete medicine and supplement list. These details help structure the history described by NIDDK. You can say: “I can get hard, but I lose firmness before sex is over. It started recently.”
Safety checks before ED medication
- Sildenafil and tadalafil are contraindicated with organic nitrates or riociguat (Viagra (sildenafil) label; Cialis (tadalafil) label).
- Common adverse effects include headache and flushing with sildenafil, and headache, indigestion and back pain with tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label).
Penile curvature changes the safety discussion too: the Cialis (tadalafil) label advises caution with anatomical deformation of the penis, such as Peyronie’s disease. Give the prescriber your full medicine list; see ED medication and nitrates.
Talk to a licensed provider about your symptoms
If your symptoms fit ED and none of the red flags apply, an online visit is one way to start. At Mendoxa, independent licensed providers review your symptoms, health history and medicines and decide whether treatment is appropriate. Prescription required.
Learn about ED treatment options before deciding whether to start a visit. An assessment comes before any prescribing decision. If you are considering Mendoxa, its compounded tablet is not FDA-approved.
FAQ
What are the early signs of erectile dysfunction?
Usually a pattern of erections that are harder to get, less firm or fade sooner than before (MedlinePlus). NIDDK lists getting an erection sometimes but not every time you want sex as a symptom.
When should I see a doctor for ED?
Arrange a visit if erection trouble keeps returning. MedlinePlus specifically advises seeing a provider when it happens more than 25% of the time. This is not a rule that you must wait to meet before asking for help.
Can I have ED if I still get morning erections?
Yes. Morning erections do not rule out erection difficulties during sex. They make a physical cause less likely (MedlinePlus), but a clinician still needs to assess the pattern.
Is ED a sign of heart problems?
It can be. In a 2005 JAMA study of men 55 and older, new ED was linked to later cardiovascular events, and the authors advised checking cardiovascular risk factors. ED does not prove heart disease; chest pain needs a 911 call (NHLBI).
Is low sex drive a symptom of ED?
Not usually: MedlinePlus says erection problems do not usually affect sex drive, while low testosterone can reduce it. Mention both symptoms if you have them.
When is an erection a medical emergency?
When it lasts more than 4 hours, painful or not. The Viagra (sildenafil) label and Cialis (tadalafil) label both call for immediate medical care.
