Can ED Be Reversed? It Depends on the Cause

Short answer

ED can sometimes be reversed, depending on its cause; NIDDK recommends treating the underlying cause when possible. Sildenafil and tadalafil help treat erection difficulties rather than reverse the underlying cause, so treatment and a cause workup can serve different goals.

Treating the underlying cause is the starting point when possible.NIH/NIDDK: ED treatment
Lifestyle evidence has limits
17 of 55 men receiving intensive lifestyle advice reached the trial’s non-ED score, versus 3 of 55 controls, over 2 years.Esposito 2004: selected obese men
Remission can happen

Can ED be reversed? By cause

Use this table to prepare for a visit, not to diagnose yourself. NIDDK lists vascular, nerve, hormonal, medicine and emotional causes; more than one may need attention.

Can ED be reversed? By cause — evidence checked Oct 21, 2026, accessed Oct 21, 2026
CauseOutlookEvidence and limitsNext step
Lifestyle / vascular risk factorsImprovement is possible; the best trial here does not cover every vascular cause. In 110 obese men aged 35–55, intensive weight-loss and activity advice led 17/55 to an IIEF score of at least 22 versus 3/55 controls after 2 years. Diabetes, hypertension and high cholesterol were excluded (Esposito 2004).Discuss risk factors; use our lifestyle guide for practical changes.
Medication side effectsA prescriber-led change may help if a medicine contributes.NIDDK recommends reviewing current medicines and considering a change when appropriate (NIDDK treatment).Ask the prescriber before changing or stopping treatment; see lisinopril and ED.
Low testosteroneSexual function may improve; improvement is not proof of permanent reversal.A 1-year trial in 790 symptomatic men aged 65+ with low testosterone found a moderate sexual-function benefit from testosterone gel (ingredient information: testosterone label; study results: Snyder 2016).Ask whether testing is appropriate; NIDDK describes a blood test and examination.
Anxiety / psychological causesCounseling can address anxiety and stress about sex.Counseling is an option when emotional issues affect ED (NIDDK counseling guidance).Discuss the pattern with a clinician; read performance anxiety and ED.
DiabetesManagement may help; it is not a promise that existing damage will reverse.Good diabetes management may prevent and treat ED associated with nerve and circulation problems (NIDDK diabetes guidance).Coordinate diabetes and ED care; see ED and diabetes.
After prostate surgeryRecovery is possible, with substantial variation.A 2009 analysis of 22 studies reported 58% erectile-function recovery after radical prostatectomy; included men had no pre-surgery ED and at least 1 year of follow-up (Tal 2009).Ask your urologist what recovery means for your operation and follow-up.

Sources: NIDDK, Esposito, Snyder, Tal. Accessed Oct 21, 2026.

Read the numbers carefully. The lifestyle trial measured an erectile-function score, while the surgical studies used recovery definitions that varied. Neither establishes a universal chance of permanent, unaided erections (Esposito; Tal).

Can ED go away on its own?

ED is not necessarily permanent: Martin’s 2014 cohort study reported 29% remission over 5 years in eligible men. But it was observational, not a trial of doing nothing; its abstract does not establish untreated recovery.

Treat that as a reason for hope, not a reason to postpone a visit. ED can signal another health problem (NIDDK). For symptoms and the broader evaluation, see our erectile dysfunction overview.

While you work on the cause, treatment can help now

You can ask about symptom treatment while pursuing the cause. Sildenafil’s label and tadalafil’s label describe ED treatment that requires sexual stimulation, not restoration of the underlying blood vessels, nerves or hormones.

For example, the Viagra (sildenafil) label reports improved erections in 57% of treated men versus 10% receiving placebo in a diabetes study, and 43% versus 15% among radical-prostatectomy patients across trials. Those are medication-response results, not reversal rates or evidence for Mendoxa’s compounded combination.

Before a visit, write down when ED began, any medicine changes, relevant health conditions and what you want treatment to achieve. Bring that list to the clinician rather than trying to choose a cause from the table.

Safety before starting ED medication

  • The Viagra (sildenafil) and Cialis (tadalafil) labels contraindicate use with organic nitrates in any form, or with a guanylate cyclase stimulator such as riociguat.
  • The sildenafil label says to get emergency care for an erection that lasts more than 4 hours; the tadalafil label warns untreated priapism “can result in irreversible damage to the erectile tissue.”
  • Stop and get care for a sudden loss of vision in one or both eyes, or a sudden decrease or loss of hearing (Viagra (sildenafil) label).

Share your complete medicine list with the prescriber. The APOKYN (apomorphine) label is for an injected Parkinson’s medicine, not approval of an ED combination; Mendoxa’s compounded tablet is not FDA-approved. See ED and nitrates.

FAQ

Can erectile dysfunction be cured?

Some causes can improve, but a permanent cure cannot be assumed. NIDDK recommends addressing the cause when possible; symptom treatment may still be needed.

Is erectile dysfunction permanent?

Not necessarily: a 5-year observational study reported 29% remission among eligible men (Martin 2014). It does not establish your likelihood of recovery or prove that improvement happened without treatment.

Can ED go away on its own?

It may improve, but “remission” does not mean “resolved without treatment.” The Martin study did not test waiting versus care; because ED may indicate another condition, discuss it with a clinician (NIDDK).

Can losing weight reverse ED?

For some men. In a 2-year randomized trial of obese men with ED, 17 of 55 in the weight-loss and exercise group scored 22 or higher on the IIEF erectile score, versus 3 of 55 controls (Esposito, JAMA 2004). The trial excluded men with diabetes, hypertension or high cholesterol.

Do Viagra or Cialis fix ED permanently?

The Viagra (sildenafil) label and Cialis (tadalafil) label describe symptom treatment requiring sexual stimulation. They do not establish permanent reversal of the cause; both labels contraindicate use with organic nitrates.

Can ED be reversed after prostate surgery?

Recovery is possible but varies. A 2009 analysis reported 58% recovery across 22 selected studies in men without pre-surgery ED, with at least 1 year of follow-up (Tal 2009). That is not a personal prognosis; ask your urologist about your operation and whether recovery is being measured with or without treatment.
