Can allopurinol cause ED? Read the label carefully
It is a reported reaction, not a confirmed explanation for your symptoms. The allopurinol label uses the terms “impotence” and “decrease in libido” under its less-common reaction heading; it also explains that some reports were voluntary, making the true frequency and link to treatment uncertain.
The label’s <1% heading is not a measured personal risk. Do not interpret it as a reliable incidence estimate; the allopurinol label explicitly limits what can be concluded from the reports.
Write down when erection changes began and when your medicine was started or adjusted, then take that timeline to your gout prescriber. Do not change treatment to test a theory yourself. The allopurinol label explains that gout flares can occur during treatment and do not by themselves require discontinuation.
Switching is a prescribing decision: the ULORIC (febuxostat) label also lists erectile dysfunction among less common reported reactions. Review the wider causes of ED rather than assuming one medicine explains everything.
What’s proven vs what’s only associated
The strongest distinction is between a verified report and a proven cause. This table separates label findings, observed ED rates and unanswered treatment questions.
| Evidence and date | Verified finding | What it means for your decision |
|---|---|---|
| US allopurinol label · updated July 31, 2026 | Impotence is listed among less common reported reactions (allopurinol label). | Supports discussing symptoms with your prescriber; does not establish individual causation. |
| UK cohort · 2018 | 11.9 vs 10.5 ED cases per 1,000 person-years, with vs without gout; adjusted hazard ratio 1.15 (95% CI 1.09–1.21). Schlesinger et al., 2018. | An association after adjustment, not proof that gout or its medicines caused ED. |
| Systematic review and meta-analysis · Sep 15, 2026 | 11 studies overall; pooled ED risk ratio for patients with gout: 1.37 (95% CI 1.14–1.65). Mao et al., 2026. | A relative comparison between groups, not an individual probability of developing ED. |
| Frontiers review abstract · 2026 | Links high uric acid and ED epidemiologically, without establishing a uniform causal relationship. Huang et al., 2026 abstract. | The verified abstract does not establish that lowering uric acid treats ED; no ED-specific urate target is recommended here. |
Sources: allopurinol label; Schlesinger et al., 2018; Mao et al., 2026; Huang et al., 2026 abstract. Accessed Oct 25, 2026.
How to read the numbers: person-years count follow-up time; hazard and risk ratios compare groups. These figures cannot tell you whether your own erection changes came from gout, a medicine or another condition.
Viagra or Cialis with allopurinol: what the labels say
The labels do not list allopurinol. Neither the Viagra (sildenafil) label nor the Cialis (tadalafil) label names it as an interaction. That finding does not establish that a combination is suitable for you; a clinician needs your full medicine list.
The Viagra (sildenafil) label and Cialis (tadalafil) label contraindicate nitrates and riociguat, caution about blood-pressure lowering with alpha-blockers and antihypertensives, and describe increased drug exposure with CYP3A4 inhibitors. Tell the prescriber about these medicines before treatment is considered.
Can Viagra cause gout? The Viagra (sildenafil) label lists gout and hyperuricemia among trial events occurring in fewer than 2% of patients, with causality uncertain. This is a reported event, not proof that sildenafil causes gout.
Bring specific concerns to the visit after reading Viagra and the heart, Viagra, Cialis and blood pressure, or ED medication and nitrates.
What each path includes
Choose the next conversation by the problem you need answered. You do not need to prove the cause yourself before asking for an ED assessment.
| Your question | Next conversation | What to bring |
|---|---|---|
| “Did this start with my gout medicine?” | Your gout prescriber can review timing alongside the reported reactions in the allopurinol label. | Start and change dates, symptom dates, and your full medicine list. |
| “Why are erections changing?” | An ED assessment considers medical, sexual and mental health history, examination and tests as appropriate (NIDDK ED diagnosis). | Symptom pattern, medical history and any results you already have. |
| “Could an online ED visit fit?” | Mendoxa offers an online assessment. Independent licensed providers decide suitability. Prescription required. | Your gout history and medicines; ask whether the compounded option is appropriate for you. |

If you have gout and ED: what to bring to your visit
Copy this checklist into your notes. It organizes the history and medicine review described by NIDDK ED diagnosis; bring existing results rather than ordering tests yourself.
- Timeline: “My erection changes began ___; my gout medicine started or changed ___.”
- Symptom pattern: trouble getting firm, staying firm, changes in desire, and whether erections happen at other times.
- Complete medicine list: prescriptions, over-the-counter products, vitamins and supplements, with the amounts you actually take.
- Health history: blood pressure, diabetes, kidney or heart conditions, plus recent results if available.
- Previous ED care: what you tried, how you took it, and what happened.
- Your question: “Can we review possible contributors and an ED option that fits my medicines?”
Include chest-pain medicines even if used only occasionally: nitrates are contraindicated with sildenafil and tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label). For a separate change in morning erections, see no morning wood.
Safety checks before ED treatment
Ask about the exact prescription, not just whether gout is compatible with ED care. The Viagra (sildenafil) label and Cialis (tadalafil) label warn against combining their products with other PDE5 inhibitors; their separate labels do not establish the safety or effectiveness of a compounded combination.
- Get immediate medical help for an erection lasting more than 4 hours (Viagra (sildenafil) label; Cialis (tadalafil) label).
- For sudden vision loss or sudden decrease or loss of hearing, the Viagra (sildenafil) label and Cialis (tadalafil) label direct patients to seek medical attention immediately.
- Common effects include headache and flushing with sildenafil, and headache, indigestion and back pain with tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label).
- The APOKYN (apomorphine) label, for an injectable Parkinson’s product, contraindicates 5HT3 antagonists such as ondansetron because of reported profound hypotension and loss of consciousness.
Bring your gout history to an online ED visit
Make the visit about the erection problem you want help with. Tell the provider what changed, what you take and what you want treatment to address. Independent licensed providers review suitability. Prescription required.

FAQ
Does allopurinol cause erectile dysfunction?
The allopurinol label lists impotence among less common reported reactions, but says frequency and causality cannot always be established. A label listing makes it worth discussing; it does not prove the medicine caused your ED. Bring your symptom timeline to your prescriber without changing treatment yourself.
Can gout cause erectile dysfunction?
Gout is associated with ED, but association does not establish cause. The Schlesinger et al., 2018 cohort found higher ED incidence with gout, and the Mao et al., 2026 meta-analysis also found increased risk. Shared conditions include high blood pressure, diabetes and kidney disease (NIAMS gout overview; NIDDK ED causes).
Can I take Viagra with allopurinol?
The Viagra (sildenafil) label does not list allopurinol as an interaction, but that does not clear you personally. Sildenafil is contraindicated with nitrates and riociguat and needs caution with alpha-blockers and blood pressure medicines, so a prescriber should review your full medicine list.
Is Cialis safe if I have gout?
Gout is not a listed contraindication in the Cialis (tadalafil) label, and the label does not name allopurinol. Suitability depends on your heart health and other medicines: tadalafil is contraindicated with nitrates and riociguat and may add to the blood-pressure-lowering effect of antihypertensives.
Can Viagra cause gout?
The Viagra (sildenafil) label lists gout and hyperuricemia among trial events seen in fewer than 2% of patients, but describes causality as uncertain. That is not proof that sildenafil causes gout; report any apparent pattern to your prescriber.
Will lowering uric acid fix my ED?
That benefit is not established by the Huang et al., 2026 abstract. Its verified abstract supports an association without a uniform causal relationship and describes treating high uric acid and ED according to their established indications. No ED-specific uric-acid target is recommended here; discuss your gout care and ED symptoms with the relevant prescriber.

How we wrote this
Written by the Mendoxa editorial team · Sources: DailyMed/FDA. Medical facts on this page come from the FDA-approved labels on DailyMed and Drugs@FDA; each drug name links to its label. We checked the linked sources on Oct 25, 2026, using the allopurinol label displayed as updated July 31, 2026. We separated reported reactions from observational findings; neither establishes the cause of an individual reader’s ED. The Huang et al., 2026 abstract was accessible as an abstract only, so we do not quote unavailable full-text sections. Study findings are not evidence that the Mendoxa compounded tablet treats gout or lowers uric acid.
