Does Cialis help with PE? What the labels and guideline say
The key question is what you mean by “lasting longer.” The Cialis (tadalafil) label covers erection problems and BPH, not delaying ejaculation; the Viagra (sildenafil) label covers erection problems, not PE.
The AUA/SMSNA ejaculation guideline names PDE5 inhibitors among drugs used for PE but does not include them in its first-line PE drug recommendation. It specifically recommends treating coexisting ED according to ED guidance. That gives an ED assessment a clear purpose when firmness is also a problem, without promising that an erection medicine will resolve PE.
Start with our tadalafil guide if your concern is erection support, or use the distinction below to prepare for a clinician’s questions.
Is it PE or is it ED?
Ejaculating quickly and losing the erection quickly are different problems that can look alike. The AUA/SMSNA ejaculation guideline defines lifelong premature ejaculation as poor ejaculatory control, associated bother and ejaculation within about 2 minutes of penetration, present since first sexual experiences. Acquired PE is a marked drop from your previous timing, with poor control and bother.
A 2024 comparison of international PE guidelines reports that some men intensify stimulation to finish before losing their erection, and that ED and PE can be confused. Rushing can therefore be a clue to erection difficulty; it does not establish a diagnosis.
- Firmness fades before ejaculation: describe when it happens and whether you rush to finish.
- Firmness holds but ejaculation feels too early: describe your control, timing, and how much it bothers you.
- Both happen: tell the clinician about both rather than choosing one label yourself.
For the firmness pattern, read losing an erection during sex and can’t stay hard. If worry is part of the experience, our performance anxiety and ED guide can help you describe it.
What each option is approved for
The table separates FDA label indications from what a US guideline lists for PE. It is not a dosing guide; PE medicines need a prescriber’s assessment.
| Option | What the label or guideline says | Role for premature ejaculation |
|---|---|---|
| CIALIS (tadalafil) | Indicated for ED, the signs and symptoms of BPH, and ED with BPH (Cialis (tadalafil) label) | PE is not an approved indication |
| VIAGRA (sildenafil) | Indicated for the treatment of ED (Viagra (sildenafil) label) | PE is not an approved indication |
| Daily SSRIs (antidepressants) | Named first-line PE pharmacotherapy (AUA/SMSNA ejaculation guideline) | Off-label for PE in the US, per the AUA panel; a prescriber decides whether and how |
| On-demand clomipramine or dapoxetine (where available) | Named first-line PE pharmacotherapy (AUA/SMSNA ejaculation guideline) | Off-label for PE in the US, per the AUA panel |
| Topical penile anesthetics | Named first-line PE pharmacotherapy; the guideline gives lidocaine, prilocaine or benzocaine as examples (AUA/SMSNA ejaculation guideline) | Listed for PE by the guideline; not an ED treatment |
Sources: Cialis (tadalafil) label; Viagra (sildenafil) label; AUA/SMSNA ejaculation guideline (2020). The AUA panel describes its PE drug options as off-label. Accessed Oct 29, 2026.
For erection problems, compare sildenafil vs tadalafil. The Cialis (tadalafil) label and Viagra (sildenafil) label both require sexual stimulation to have an effect.
What studies show about tadalafil and sildenafil for PE
Some pooled studies found benefit, but the tadalafil trial below did not beat placebo. These are different questions: whether a drug class helps across mixed trials, whether adding a drug helps, and whether tadalafil alone helps lifelong PE. PE is not an indication in the Cialis (tadalafil) label or Viagra (sildenafil) label.
| Study | What was studied | Abstract finding | What it cannot tell you |
|---|---|---|---|
| Abou Faddan · 2022 | 55 men with lifelong PE; daily tadalafil versus placebo; follow-up at 1 and 3 months | No statistically significant difference from placebo in ejaculation timing or PE questionnaire scores | Small, single-blind study; not proof of no possible benefit |
| Martyn-St James · 2017 | 15 randomized trials of PDE5 inhibitors, alone or with an SSRI | Pooled findings favored PDE5 inhibitors over placebo and combination therapy over an SSRI alone | Results varied substantially; most trial methods were of unclear quality; class findings are not tadalafil-only results |
| Bai · 2015 | 6 publications; 971 patients; SSRI plus PDE5 inhibitor versus either alone | Combination therapy improved time to ejaculation; reported adverse events were 37.5% versus 25.63% with single-drug treatment | Combination results cannot establish benefit from tadalafil alone |
Sources: PubMed abstracts linked in each row; Cialis (tadalafil) label; Viagra (sildenafil) label. IELT means time from vaginal penetration to ejaculation. Adverse-event percentages are the review’s reported values, not a prediction of your risk. Accessed Oct 29, 2026.
What this means for your decision: do not turn a combination-study result into a reason to add medication yourself. Ask the clinician to assess erection quality and ejaculation concerns separately, then discuss the relevant evidence.
Does tadalafil make you last longer?
A longer window for erection response is not the same as delayed ejaculation. The Cialis (tadalafil) label reports improved erectile function versus placebo up to 36 hours after as-needed use and a mean terminal half-life of 17.5 hours in healthy subjects. Sexual stimulation is still needed; neither figure measures how long sex or an erection will last.
For erection-treatment timing, read how long Cialis lasts and daily tadalafil. For a comparison of erection medicines, see sildenafil versus tadalafil. (Viagra (sildenafil) label).
What each path includes
Match the visit to the problem. Use this to decide what to raise first; it is not a self-treatment plan.
| What is happening | Likely starting point | What it includes | Boundary |
|---|---|---|---|
| I lose firmness, so I rush to finish | ED assessment (NIDDK) | History, medicines, and an ED prescription if suitable (Cialis (tadalafil) label; Viagra (sildenafil) label) | Prescription required; nitrates are contraindicated (Cialis (tadalafil) label; Viagra (sildenafil) label) |
| I stay firm but ejaculate sooner than I want, with poor control and bother | PE discussion with a clinician (AUA/SMSNA ejaculation guideline) | History, focused exam, first-line PE options and behavioral approaches | PE drug options are off-label in the US, per the AUA panel |
| Both problems at once | Assess both; treat coexisting ED according to ED guidance (AUA/SMSNA ejaculation guideline; 2024 review of international PE guidelines) | Discuss both concerns and how to follow up | Do not combine prescriptions on your own |
| I want to know whether Mendoxa fits my ED | Mendoxa online visit | Review by Independent licensed providers; Prescription required | For ED only; the compounded tablet is not FDA-approved |
Sources: AUA/SMSNA ejaculation guideline; 2024 review of international PE guidelines; NIDDK ED diagnosis; Cialis (tadalafil) label; Viagra (sildenafil) label; FDA compounding Q&A; Mendoxa product information in the ED intake. Accessed Oct 29, 2026.
Before the visit, write down what happens first—loss of firmness or ejaculation—whether this has always happened or is a change, and what medicines you use. If an erection medicine has already disappointed you, read ED pills not working and bring those details.

Safety checks before ED medication
- Sildenafil and tadalafil are contraindicated with organic nitrates or riociguat (Viagra (sildenafil) label; Cialis (tadalafil) label).
- An erection lasting more than 4 hours needs immediate medical care (Viagra (sildenafil) label; Cialis (tadalafil) label).
- Stop and seek care for sudden vision loss or sudden decrease or loss of hearing (Viagra (sildenafil) label; Cialis (tadalafil) label).
- Common adverse effects include headache and flushing with sildenafil, and headache, indigestion and back pain with tadalafil (Viagra (sildenafil) label; Cialis (tadalafil) label).
- The APOKYN (apomorphine) injection label contraindicates use with 5HT3 antagonists such as ondansetron and warns about nausea and fainting.
Do not add another ED medicine to an existing treatment on your own: the Viagra (sildenafil) label and Cialis (tadalafil) label say combinations with other PDE5 inhibitors or ED therapies have not been studied and advise against such use. See ED medication and nitrates.
If ED is the problem you want treated now
If erection loss is making you rush, start by having that assessed. An online ED visit gives you a place to describe the firmness problem and ask whether ED care fits. Independent licensed providers assess suitability. Prescription required. If ejaculation timing is the concern while firmness holds, arrange a PE discussion with a clinician.
The Mendoxa compounded tablet is not FDA-approved. Read about the compounded combination ED tablet before deciding whether to begin the ED visit. Costs below apply if prescribed.

FAQ
Can Cialis help with premature ejaculation?
It has been studied, but PE is not an approved use in the Cialis (tadalafil) label. A 2022 placebo-controlled study in lifelong PE found no statistically significant benefit over placebo. The AUA/SMSNA ejaculation guideline recommends treating ED when it coexists with PE.
Does Viagra help with premature ejaculation?
The Viagra (sildenafil) label lists ED only. A 2017 meta-analysis found PDE5 inhibitors outperformed placebo for ejaculatory latency, but most trials were of unclear quality and results varied widely; a clinician should assess whether ED is the underlying issue.
Does tadalafil make you last longer?
The Cialis (tadalafil) label describes erection benefit up to 36 hours after as-needed use, not 36 hours of sex or an erection. Its mean half-life is 17.5 hours in healthy subjects; that does not establish a delay in ejaculation.
Can you take Cialis with an SSRI for PE?
Some studies combined them, and a 2015 meta-analysis found better latency with the combination but more adverse events. That is a prescriber decision; do not combine medicines on your own, and review the Cialis (tadalafil) label with your clinician.
How do I know if it is PE or ED?
Tell the clinician whether firmness fades before ejaculation or firmness holds but ejaculation feels too early. The AUA/SMSNA ejaculation guideline defines PE using poor control and bother as well as timing; lifelong PE includes ejaculation within about 2 minutes of penetration since sexual debut, while acquired PE involves a marked reduction from previous timing. This distinction needs assessment, not a stopwatch alone.
Can I get Cialis online for premature ejaculation?
The Cialis (tadalafil) label does not list PE as an approved use. If you also have trouble maintaining firmness, an online ED visit can assess that concern. Independent licensed providers determine suitability. Prescription required. Mendoxa’s compounded tablet is not FDA-approved; PE alone needs a clinician’s discussion.

We compared label indications, the AUA/SMSNA guideline and NIH-hosted study abstracts, accessed Oct 29, 2026. The evidence table separates a tadalafil trial from drug-class meta-analyses: a pooled result does not establish how a particular tablet will work for you.
